症例は41歳，女性，1995年10月23日に右外鼠径ヘルニアの卵管嵌頓にて腹腔鏡下鼠径ヘルニア修復術が施行された．1996年8月，1997年3月，1998年1月に右鼠径部の水腫を発症したが，いずれも水腫の穿刺吸引にて軽快した．1999年8月に再度，右鼠径部の水腫を発症し，1999年9月2日に外科的に残存末梢側ヘルニア嚢は，水腫の再発を認めていない．一般的に残存ヘルニア嚢の水腫は，自然消失するか，穿刺吸引にて容易に軽快するといわれている．しかし，今回のように外科的な治療が必要な症例も存在する．このような合併症を予防するためには，精索やヘルニア嚢周囲の過度の剥離は避け，残存ヘルニア嚢の開放を心掛けるべきである．
A 41-year-old woman complaining of right indirect inguinal hernia with incarcerated oviduct underwent laparoscopic herniorrhaphy on October 23, 1995. Subsequently, hydrocele developed in August 1996, March 1997. and January 1998. In August 1999, hydrocele developed again, and the residual distal hernia sac was re-moved surgically on September 2, 1999. Hydrocele has not developed since the second operation. Generally, hydrocele from the remaining hernia sac resolves spontaneously, or responds to simple aspiration. But this case required the secondary operative correction.
