腹腔鏡下胆嚢摘出術は，手術侵襲の軽減，早期の術後回復，周術期におけるQOLの獲得を可能にする点でクリニカルパスの最も適応のある手技の1つと考えられる．当科では，在院日数の短縮化，チーム医療の確立，医療に対する患者の満足度の獲得にとって，有意義な手法として位置付けている．術前の諸検査，評価は外来で行っている．術式は循環・呼吸管理のしやすい“吊り上げ式腹腔鏡下胆嚢摘出術”を採用している．ドレーン挿入は原則として行わない．抗生物質は術後1回のみ静注．胃カテーテル，尿道バルーンは術後1日目に抜去し，早期離床をはかっている．術後1日目に飲水を開始し，その後，経口食に移行する．術後3〜4日目には退院とし．抜糸は術後1週間目に外来にて行っている．
Laparoscopic cholecystectomy is one of the most useful surgical techniques for cholecystolithiasis because of its minimal invasiveness. rapid recovery and QOL of the patient. We consider the perioperative management of this procedure to be a good indication for “clinical paths” adaptation. The clinical paths contribute to shortening of hospitalization, team medical treatment and the patients satisfaction with medical care. We assess preopera-tive examinations and complications at ambulant periods.
