◆要旨：腹腔鏡下ヘルニア修復術は，腹腔鏡用と切離デバイス用，そして牽引用に各々1ポートを用いて行うことが一般的であるが，単ポートによる牽引では剝離層を確実に視認できる良好な術野展開がしばしば容易でない．筆者らは，術野展開を容易にすることを目的に，剝離したヘルニア囊にナイロン糸をかけ持続的な牽引を可能にするヘルニア囊牽引法（hernia sac-traction method：HST法）を考案した．対象はヘルニア囊の剝離に強い牽引を要する外鼠径ヘルニア症例で，本法を用いることにより良好な術野展開のもと，ヘルニア囊の境界を確実に確認することができた．本法を用いることで安定したストレスの少ないヘルニア手術が可能となると思われる．
Laparoscopic inguinal hernia repair generally requires deployment of the operative field from only one access port other than the laparoscope and the cutting device, and it is often not easy to develop a flat surgical field to confirm the optimal dissecting layer reliably. We devised a “Hernia sac-traction method （HST method）”, which applied nylon thread to the dissected hernia sac and maintained continuous traction from the port on the affected side. Its sustained traction enable to facilitate good operative field deployment. Especially in the case of large external inguinal hernia, requiring strong traction of the hernia sac for dissection, this method provides reliable confirmation of the boundary of the hernia sac under good operative field. We believe that HST method enables stable and less stressful hernia surgery.
