腹腔鏡下鼠径ヘルニア修復術を行ったinterparietal herniaの1例

Interparietal hernia repaired by laparoscopic transabdominal preperitoneal approach : A case report

◆要旨：患者は73歳，男性．約1年前より右下腹部の膨隆を自覚して当院を受診した．右外鼠径輪より外頭側の位置に3cm大の扁平な膨隆を認めた．造影CTではヘルニア囊は内鼠径輪より脱出し，頭外側方向に向かい内外腹斜筋間に達していた．腹腔鏡下に手術を行った．内鼠径輪にヘルニア門を認めたが鼠径管方向は盲端となり，ヘルニアは頭外側に向かっていた．CT所見と併せ，interparietal herniaと診断した．修復はI型鼠径ヘルニアに準じ，腹腔鏡下鼠径ヘルニア修復術を行った．本症例はI型鼠径ヘルニアに伴ったinterparietal herniaであり，腹腔鏡下鼠径ヘルニア修復術は診断の確定とヘルニア門の確実な修復に有用であった．

　A 73-year-old man presented with right lower abdominal swelling. The swelling was 3cm in size and located on the laterally temporal side of the right inguinal ring. Enhanced CT revealed that the hernia had prolapsed from the internal inguinal ring and reached between the internal and external oblique muscles. Laparoscopic operation revealed that the hernia prolapsed from the internal inguinal ring and extended upper laterally. The hernia was repaired by laparoscopic transabdominal preperitoneal approach. This approach was useful for diagnosis and repair.
