◆要旨：食道癌に対する腹臥位胸腔鏡下食道切除は近年急速に普及している．しかし導入当初から左反回神経周囲の視野展開の困難さが指摘されている．狭い視野での無理な操作は左反回神経麻痺の原因にもなりうる．筆者らは2009年の導入後より反回神経麻痺を回避するための手技の改良を重ね現在に至っている．106recL最上部までの郭清操作において郭清終了まで食道を離断せずに反回神経に均等かつ適度な緊張を保ちながら，1本ずつ尾側から食道枝を切離することによって郭清を進めるため反回神経を過度に牽引することがない．この方法を定型化してからは反回神経麻痺の頻度が著明に低下した．筆者らの手技の工夫について報告する．
Recently, in many institutes in Japan, thoracoscopic esophagectomy in the prone position has been employed for radical esophagectomy for patients with esophageal cancer. However, lymphadenectomy along the left recurrent laryngeal nerve has been thought to be a burdensome procedure because of the difficulty to obtain good operative field at the left upper mediastinum. We performed lymphadenectomy of this part without dividing the esophagus throughout this procedure. It allows the adequate tension of the left recurrent laryngeal nerve, resulting in avoiding nerve palsy. In fact, the incidence of recurrent nerve palsy was obviously low after the standardization of this procedure. We report our surgical procedures of left upper mediastinal lymphadenectomy to avoid left recurrent laryngeal nerve palsy.
