ハーモニック・スカルペル＊（coagulating shears：CS）を用いた肝切除術の経験

Technique of Hepatic Resection Using Harmonic Scalpel (Coagulating shears)

　ハーモニック・スカルペル^＊（coagulating shears：以下，CS）の内視鏡下肝切除術への応用の可能性を検討するため，6例の開腹下肝切除術でCSを用いた．肝切離端からの出血が少なく，胆汁漏出も認めず，比較的低温であるため周囲肝組織への影響も少なかった．術後出血，胆汁漏出，断端腫瘍は経験せず，肝切除器械として有用と思われたが，アクティブブレードの先端部に太い脈管が存在する場合や脈管側壁を把持した場合には，同部からの出血を来した．現在のブレードの形状では表面から観察し得ない深部に死角が生じるため，術中超音波検査で肝切離予定面の脈管位置を検索し把握することが重要で，結紮すべき脈管の手前に“入れ墨”をしておくなどの工夫が必要と考えられた．

We have performed 6 cases of hepatic resection under laparotomy using Harmonic Scalpel (Coagulating shears, CS) , and examined its possible future usage in laparoscopic hepatic resection. By using CS, bleeding from the cutting surface was minimized, no bile leakage was encountered, and thermal damage was also minimized. No complication in association with CS, such as postoperative bleeding, bile leakage, and adscess formation at cut ends, has been found during the post-operative courses. Therefore CS was considered as one of the useful devices in the hepatic resection.
