　最近になって，血管外科分野で内視鏡下手術が少しずつ取り入れられつつある．われわれは，内視鏡下後腹膜経由腰部交感神経切除術を1995年10月から12月の3か月間に，3症例（1症例は両側）の虚血性足趾に対し行い，比較的良好な結果を得た．後腹膜腔を剥離し，4本のトロッカーを挿入し，従来の切開法と同様の手術を行った．手術時間は67〜95分であった．術後合併症もなく，従来の切開法に比べて術後疼痛が軽微で，術後回復が早く，腰部交感神経切除術も内視鏡下手術の1つになり得ると思われる．

In recent vascular surgery, endospic approach has been employed in. We performed an endoscopic retroperitoneal lumbar sympathectomy (ERLS) in three patients (one patient had bilateral ERLS) from Oct. 1995 to Dec, 1995 and obtained relatively good results. As a surgical technique, retroperitoneal space was dilated with a balloon dilator (Origin company), and four trochars were placed into the retropertoneal space. After that, similar procedure was followed for open lumber sympathectomy. The operation time was 67 to 95 minutes. Postoperatively, no major complications were observed.
