熱傷面積が20％ total body surface area（TBSA）を超える重症熱傷患者では，“hypermetabolism”と呼ばれる全身性の代謝の高度亢進をきたし，同化より異化が上回り，肝障害や筋肉量の低下をもたらすことが古くから知られている。この高度飢餓状態とも言える状況に対する治療介入の1つが栄養療法である。これまでの研究結果から受傷後24時間以内に経腸栄養を開始することが求められているが，必要エネルギー量や微量栄養素の必要性については，いまだ決着はついていない。海外ではhypermetabolismを抑制する目的でβ遮断薬，タンパク質同化ステロイド，成長ホルモン製剤投与の研究が進んでいる。

It has long been known that patients with more than 20％ total body surface area burns develop a systemic metabolic derangement called “hypermetabolism”, in which catabolism exceeds anabolism, resulting in liver damage and loss of muscle mass. Nutritional therapy is one of the interventions to improve the resulting state of starvation. Studies have shown that enteral nutrition should be initiated within 24 hours after injury. The exact energy requirements and micronutrient needs have not yet been determined. In other countries, treatment with beta-receptor blockers, anabolic steroids, and growth hormones has been studied to suppress hypermetabolism.
