4．人工呼吸器関連肺炎予防—これまでとこれからの常識

人工呼吸器関連肺炎（VAP）はICUで最も多い医療関連感染症である。罹患により患者予後に影響し，医療費も増大するため，その予防は大いなるテーマとなっている。人工呼吸器の使用をなるべく回避すること，短くすることが最大の予防であるが，さまざまな重症病態のために人工呼吸器が必要になってしまった場合には，いくつかのVAP予防策をとることが必要である。これまでに推奨されている予防策はバンドルとしてまとめて行われるのが通常だが，本稿では改めてそれぞれの項目について記した。また，カフ上吸引付き気管チューブの使用やカフ圧管理，早期気管切開，選択的消化管除菌，栄養投与方法，プロバイオティクスの使用など，バンドルに含まれるもの以外についても，最近の新たな知見を含めて解説した。バンドルの順守とともに各項目を個々の病態に合わせて実施していくことが，予後改善につながるのだろう。

Ventilator-associated pneumonia （VAP） is the most common healthcare-associated infection in the ICU, and its prevention is a major topic of concern because of its impact on patient outcomes and increased healthcare costs. The best prevention is to avoid or shorten the time on the ventilator as much as possible, but when mechanical ventilation is necessary due to the patient's respiratory status, several prophylactic measures should be taken. Many of the recommendations are grouped together as a prevention bundle, and this chapter describes these items. In addition to those included in the bundle, the use of an endotracheal tube with subglottic suctioning, cuff pressure management, early tracheostomy, selective gastrointestinal decontamination, nutritional administration methods, and the use of probiotics are described, including recent findings. Adherence to the bundle and implementation of each item in accordance with the individual patient's condition may lead to improved outcomes.
