高所や水中等の特別な環境で診療することは少ないが，急性高山病や減圧症，nitrogen narcosis，shallow water blackoutはICU管理を行っていると遭遇することはあり，それらの診療を行う際には病態生理学を知っておく必要がある。高所環境への急激な曝露に対する呼吸順応がどのような過程で起こるのか，なぜ急性高山病が発症するのか，高地住民のような慢性的な高所曝露ではどのような変化が起きているのかを解説する。また水中における呼吸生理がどのように変化するのか，減圧症やnitrogen narcosis，shallow water blackoutがどのように発症するのかも解説する。

Although treatment is rarely performed in special environments such as at high altitudes or underwater, acute mountain sickness, decompression sickness, nitrogen narcosis, and shallow water blackout may be encountered in critically ill patients. It is necessary to know the pathophysiology when treating patients with these conditions which arise in special environments. In this paper, we explain the process of respiratory adaptation to sudden high-altitude exposure, why acute altitude sickness develops, and what changes occur after chronic high-altitude exposure, such as in highlands. This paper also explains how respiratory physiology changes in water and how decompression sickness, nitrogen narcosis, and shallow water blackout develop.
