5．免疫調整栄養剤は投与すべきなのか？—新たなエビデンスの確立を目指して温故知新

重症患者に対する栄養療法において，免疫調整栄養剤と称されたアルギニン，グルタミン，ω3系脂肪酸による予後改善効果が検討されてきた。これらの栄養素には，その効果が臨床試験の結果から有効であると評価された時期もあったが，2018年現在，いずれも患者予後には結びつきにくいと考えるのが一般的となっている。しかし，これまでの流れを振り返ることは，近年の栄養学の歩みや，今後どのような方向性で研究が進む可能性があるかを考えるにあたって重要である。

　本稿では，各栄養素に分けて，①どのような病態生理学的根拠で推奨され，②代表的な臨床試験にはどのようなものがあり，③今後どのような点について検証すべきか，について論じる。

In nutritional therapy for critically ill patients, immunonutrition, such as arginine, glutamine, and omega-3 fatty acids have been studied to determine if their administration improves clinical outcomes. Although some clinical studies favored these nutrients, as of 2018, however, the clinical effect of these nutrients has not been associated with improved outcomes. It may still be important to review the progress of this topic to discuss future studies. This paper will cover how each of these nutrients are studied in terms of their pathophysiological aspects, what major clinical trials prove, and possibilities for future studies.
