2020年のBD/DNC（brain death/death by neurologic criteria）は脳死の世界初のコンセンサスで，深昏睡，脳幹反射の消失，自発呼吸消失で定義される脳機能の完全かつ永続的な喪失状態である。無呼吸テストではベースライン時の低血圧，A-aDO₂開大，低酸素血症，アシデミア，年齢はリスク因子であり，呼気終末陽圧（PEEP）下で施行することが勧められる。日本では脳死とされうる状態においても脳波をとることが義務づけられている。法的脳死判定における無呼吸テスト（成人）および脳波検査は1回とすることは妥当であり，今後省令改正を要望する。

Abstract

The 2020 Brain Death/Death by Neurologic Criteria is the global consensus for brain death, a state of complete and permanent loss of brain function defined by deep coma, loss of brainstem reflexes, and loss of spontaneous breathing. Risk factors for apnea include hypotension at baseline, A-aDO₂ opacification, hypoxemia, acidemia, and age. The test must be performed using positive end-expiratory pressure . In Japan, EEG is mandatory even under conditions that could be considered brain death. A single apnea test and electroencephalogram are reasonable methods for the diagnosis of brain death.
