I．はじめに

　頭蓋内動脈瘤の破裂による鼻出血の症例は稀であるが，診断が遅れると大量の鼻出血を招き致命的な結果となる場合がある。動脈瘤性鼻出血の報告ではそのほとんどが頭部外傷後に発生し，受傷時に頭蓋底骨折や視力障害を伴い，III・IV・V・VIの脳神経障害を合併していることが多い。

　われわれは海綿静脈洞部に発生した動脈硬化性と考えられる内頸動脈瘤のために反復性大量鼻出血を起こした症例を経験した。この症例は頭部外傷の既往がなくしかも上記の脳神経障害を伴わず，大量の反復性鼻出血が唯一の特徴的所見であった。このような症例はきわめて稀であるので，ここにその経過を報告し若干の考察を行う。

　The aneurysmal epistaxis following a head trauma usually accompanies with basilar skull fracture, visual loss and cranial nerve palsies.

　We report a case of recurrent severe epistaxis from non-traumatic aneurysm without such symptoms. Carotid angiography demonstrated an aneurysm in the left cavernous portion of the internal carotid artery extending to the sphenoid sinus. The aneurysm was clipped and there was no recurrence of epistaxis after the operation.
