外傷性内頸動脈瘤による反復性大量鼻出血

SEVERE RECURRENT EPISTAXIS CAUSED BY TRAUMATIC ANEURYSM OF THE INTERNAL CAROTID ARTERY

　Ⅰ．緒言

　近年交通事故の増加によつて，頭部，顔面部の外傷例は激増してきている。開放損傷では頭蓋内外の出血，すなわち内外頸動脈領域の血管損傷があるのは当然であるが，一見閉鎖性頭部外傷と思われる場合で大量鼻出血をみる例を経験することがある。これらの大部分は前頭蓋窩，蝶形骨洞などの頭蓋底骨折により，前篩骨動脈，翼突口蓋動脈損傷などの動脈性出血であることが多く，時に静脈洞損傷による静脈性出血もみられる。いずれにしろ，この場合は受傷直後より鼻出血があるために，出血巣は精査され，止血操作が加えられている。これに反して受傷により内頸動脈瘤を形成し，しばらくの時日を経た後に，反復する大量鼻出血発作を起こす例が稀にあることが知られている。外傷後の大量鼻出血例は今後益々増加することが予想され，その際，動脈瘤形成によるものも存在することを考慮しておかねばならないが，耳鼻咽喉科領域においては，この種の報告がきわめて少ない。

　われわれは交通事故によると思われる内頸動脈瘤からの重篤な鼻出血例を2例経験したので，ここに報告し，併せて若干の文献的考察を加えた。

　Two cases of epistaxis, both severe and recurrent, which were apparently due to the formation of aneurysm of the internal carotid fol-lowing a penetrating head injury, are reported.

　In the first case a man, aged 23, suffered of severe epistaxis that began on the tenth day after the head injury. The bleeding continued without abatement until a vain attempt of ligation of the external carotid was made. However the hemorrhage was finally controlled by trapping and muscle imbobilization.

　In the second case a man, aged 59, suffered a severe epistaxis that occurred 14 days after the head trauma. The hemorrage was brought under control by ligation of the internal carotid artery in the neck without any adverse consequence.
