I．はじめに

　鼻咽腔血管線維腫はほとんどが思春期男子の鼻咽腔に発生する比較的稀な腫瘍である。病理組織学的には血管と線維性結合組織に富み被膜をもたない良性腫瘍像を示すが，拡張的増殖と易出血性のため放置すると種々の重篤な症状を呈するに至る。さらに摘出が容易でなく再発しやすいことから臨床的には悪性腫瘍に準じて取り扱われることがある。原因は思春期におけるホルモン代謝異常や炎症性産物であろうとされている。

　最近私たちは上咽頭に発生し頭蓋底の骨破壊を伴う比較的大きな鼻咽腔血管線維腫の1例を経験し，経口蓋法と術前動脈塞栓術，軽度低体温法，低血圧法を用いて外科的に完全摘出することにより治癒させた。その臨床経過に外科的摘出法についての文献的考察を加えて報告する。

　A 15-year-old boy had been suffering from intermittent epistaxis and nasal obstruction for 2 years.X-ray examinations showed extention of a tumor into the right ethmoid, sphenoid sinuses and the skull base. After preoperative arterial embolization, he underwent surgical resection of the tumor by transpalatine approach under general anesthesia with the controlled hypothermia and hypotention. The tumor was successfully removed and there was no evidence of residual or recurrent tumor for 1 year. The surgical approaches to the nasopharyngeal angiofibroma were discussed.
