下鼻甲介に発生した若年性血管線維腫の1症例

A juvenile Angiofibroma of Inferior Turbinate:Report of a Case

　はじめに

　若年性血管線維腫は思春期の男性に好発する良性腫瘍で特異的に鼻咽腔に発生することが知られているが，その拡張的増殖と易出血性のため，悪性腫瘍に準じて取り扱われている。また，鼻咽腔以外に発生することは極めて稀とされている。今回われわれは，15歳女性で下鼻甲介に広基性の茎をもち，鼻腔内より上顎洞内へ進展した血管線維腫を経験し，術前の栄養動脈塞栓術と経上顎洞的手術療法を用いて腫瘍を完全に摘出しえたので若干の文献的考察を加え報告する。

Juvenile angiofibroma is benign tumor that almost occurs in adolescent males. However it should be managed as malignant tumors because of locally expansive nature and high vascularity. In the majority of cases, they developed in the nasopharynx. Recently, we have treated a 15-year-old female with an extremely rare type of angiofibroma of the inferior turbinate and it prog-ressed from the nasal cavity to maxillary sinus. After selective embolization of the sphenopalatine and facial arteries through angiography, the tumor was completely removed through a perimaxillary approach. The patient showed no recurrence.
