鼻咽腔血管線維腫の1例とその手術法に関する考察

A CASE OF JUVENILE ANGIOFIBROMA

　I．緒言

　鼻咽腔血管線維腫の手術は，多くの耳鼻咽喉科医にとって，できうるなら回避したい疾患ではないだろうか。その理由は，上咽頭が奥まった視野の得にくい部位であり，腫瘍そのものが血管に富み，手術時大量の出血を伴う恐れがあるからである。

　筆者らは最近，比較的大きな鼻咽腔血管線維腫に遭遇し，経上顎洞法および経口蓋法の併用により，完全摘出できたので，その手術法について考察し報告する。

　A 22-year-old male patient with juvenile angiofibroma was reported. The tumor was removed by combined approach of transpalatine and transmaxillary exposure of the nasophanynx.

　The methods of approach to the nasophanynx was discussed, and it was emphasized that carotid arteriogram was helpful in delineating feeding vessels for reducing hemorrhage during the operation.
