I．はじめに

　慢性鼓膜炎の経過が永びくと外耳道に結合織の増生が起こり，鼓膜が数ミリメートルの厚さになり外耳道狭窄の所見を呈するようになると成書に記載^1）があり，最近Katzkeら^2）はこれをpostinflammatory medial meatal fibrosisと呼んで一つの疾患単位と考えることを提唱している。わが国での慢性鼓膜炎の症例をみているとこうした増殖性変化はきわめて少なく，"増殖性変化は起こりにくい"と著者らはかつて報告した^3）。

　こうした稀な増殖性変化を伴った両側の難治性慢性鼓膜炎を最近経験し，また同じ時期に皮膚科で治療中の3例の全身性水疱性疾患の患者の鼓膜に類似した鼓膜の変化を認めた。慢性鼓膜炎の原因は不明であるが，皮膚疾患の症例で似たような変化を認めたことは原因究明の手がかりになりはしまいかと考え検討したのでここに報告したい。

　Excessive fibrous hyperplasia of the tympanic membrane and the bottom of the external ear canal was found in both ears of a 28-year-old female with chronic myringitis. These changes were correspondto the findings reported by Katzke et al. as the postinflammatory medial meatal fibrosis. Movable tympanic membrane and restoration of hearing were gained after surgical removal of proliferated subcutaneous connective tissue and free skin grafting. No deposition of antibody was proved by the direct and indirect antiskin antibody test in the external ear canal skin. Similar changes of the tympanic membrane in cases with bullous pemphigoid, pemphigus vulgaris and epidermolysis bullosa hereditaria were also reported.
