Gradenigo症候群を呈した慢性中耳炎症例

A Case of Chronic Suppurative Otitis Media Representing Gradenigo's Syndrome

I．はじめに

　近年の化学療法の進歩に伴って中耳炎の合併症は減少し錐体尖炎も稀なものとなっている。したがって中耳炎が存在しその合併症である錐体尖炎による三叉神経刺激痛と外転神経麻痺をトリアスとするGradenigo症候群も非常に稀なものとなったようである^1）。

　筆者らは慢性中耳炎の急性増悪期にGradenigo症候群を呈した症例を経験し，化学療法により症状を消失せしめた後に鼓室形成術を施行し良好な結果を得たのでその詳細を若干の文献的考察を加えて報告する。

　A case of Gradenigo's syndrome accompanied by acute exacerbation of chronic suppurative otitis media with cholesteatoma is reported. Trigeminal pain and paralysis of the abducens nerve were disappeared after intensive chemotherapy. After cholesteatoma and infection were completely eradicated, tympanoplasty (modified type III) was performed using ceramic partial ossicular replacement prosthesis. Hearing was improved and no recurrence of infection was observed.

　We will emphasize that the adequate chemotherapy and operation, especially tympanoplasty, can be indicated for Gradenigo's syndrome.
