眼症状を呈した副鼻腔アスペルギルス症の1例

Aspergillosis of the Maxillary Sinus with Orbital Complication: Report of a Case

I．はじめに

　最近市村ら^1）は鼻・副鼻腔真菌症に関し，自験例13例を含む国内報告例66例と国外報告例372例の検討を行い，本疾患に合併する眼障害の頻度が本邦ではかなり低率であることを明らかにしている。

　われわれの教室で昨年右眼球突出と右眼瞼・頬部腫脹を主訴に来院し，外来検索期間中に視力低下をきたした副鼻腔アスペルギルス症を経験し，その眼窩内病変が治療とともに変化する様子をcomputed tomography（以下CTと略す）により観察した。

　A woman, aged 74, complained of exophthalmus of the right eye and visual disturbance.

　CT showed a high density mass of the right maxillary sinus and orbit.

　There was aspergilloma in the sinus but no dehiscence of the bony wall of the orbit, then we performed Caldwell-Luc operation.

　CT revealed that the residual orbital mass increased of its size in 10 days following the operation.

　The patient was treated with 5-fluorocytosin, and had been cured clinically, but CT still revealed some abnormal shadows in the orbit 1 year post-operatively. Further careful follow up must be necessary.
