Gradenigo症候群の1例

Gradenigo's Syndrome:Report of a Case

　はじめに

　Gradenigo症候群は一般に中耳炎が存在し，これに三叉神経痛と外転神経麻痺を伴った場合と定義される^1）。多くは中耳炎の急性憎悪時に中耳の炎症が錐体尖端部に波及し，そこに限局性の炎症を起こすことに起因するといわれている。古典的な疾患の1つであるが，抗生剤の進歩，耳性頭蓋内合併症の減少によりその報告はきわめて少なくなっている。しかし，硬膜外膿瘍や髄膜炎などの合併症を起こすこともあり，その診断とさらにその治療は非常に大切である。今回われわれは，典型的なGradenigo症候群を経験したので報告する。

A 10 year-old boy presented with headache, trigeminal irritation symptoms and right abducens paralysis. On examination, the patient had acute otitis media without pain. After admission, he wasgiven antibacterial drugs and received paracent-esis of the right ear drum. Headache and trigeminal pain were improved but abducens paralysis was not improved.

Right simple mastoidectomy was performed and steroid therapy was started. After the operation, abducens paralysis was relieved. Gradenigo's syn-drome described in the present case is rare after the antibiotic era of the recent years.
