I．緒言

　鼻出血は日常臨床上しばしば遭遇する症候であるが，ほとんどは外頸動脈領域または眼動脈の分枝である前―および後篩骨動脈領域からの出血で，その処理も比較的容易である。しかしながら稀ではあるが，内頸動脈そのものからの出血も報告されている。それらは頭部外傷後に起こる外傷性動脈瘤，先天性あるいは動脈硬化性による非外傷性動脈瘤および手術時の損傷による動脈瘤よりのものに大別される。いずれの場合にも動脈瘤は内頸動脈のcavemous portionに生じ，後飾骨洞または蝶形骨洞に破裂して大出血をきたすとされている。われわれは副鼻腔炎の手術後45年を経過して大量の鼻出血を反復し，その出血が内頸動脈のcavernous portionに発生して，蝶形骨洞内に突出する仮性動脈瘤からのものであった1例を経験したので報告する。

　A 67-year-old woman was admitted to our hospital with the complaint of recurrent massive epistaxis from the right side. She had sinus surgery in the right side 45 years ago, and the inferior and middle turbinates were completely removed and the sphenoidal sinus was kept open. With CAG, bleeding was proved to be from an aneurysm at the cavernous portion of the internal carotid artery. Surgery was successfully performed by the trapping and coating.

　Otolaryngologists should keep it in mind that when rapid and severe epistaxis is observed it is probably originated from the internal carotid artery, and if the normal anatomical structures in the nasal cavity are removed and sphenoidal sinus is open, the internal carotid artery will be most susceptible to the injury.
