鼻出血にて発症した外傷性仮性内頸動脈瘤の1治療例

One Treatment Case of Traumatic Pseudoaneurysm of Internal Carotid Artery Occurred Epistaxis

　要旨　鼻出血にて発症した外傷性仮性内頸動脈瘤の1治療例を報告した。症例は53歳男性。交通事故にて搬送され，出血性ショックと左眼失明を伴っていた。輸血などにてショック状態を改善後，MRIにて外傷性仮性内頸動脈瘤の診断のもとに，脳血管写をしたところ，親血管閉塞試験にて陽性所見が得られた。受傷後6日目，Guglielmi detachable coil（GDC）による瘤内塞栓術を施行した。

　脳血管写，MRIにて6カ月間経過観察しているが，脳動脈瘤の増大はなく経過良好である。外傷性仮性内頸動脈瘤は早期診断が大切であり，MRIも有用である。治療は頸部内頸動脈結紮やtrapping，血管内治療による損傷部位の閉塞が基本であるが，親血管閉塞試験にて脳血流の低下や神経脱落症状の出現などの陽性所見が得られた場合，仮性動脈瘤の発生部位によっては瘤内塞栓術が可能な場合がある。

We reported a case of traumatic pseudoaneurysm of internal carotid artery accompanied by skull base fracture, for which Guglielmi dtachable coil(GDC)embolization surgery was very effective for recovery.

A 53-year-old man met a traffic accident and was admitted to our hospital with blindness and epistaxis accompanied with shock. Balloon occlusion test showed a positive sign of traumatic pseudoaneurysm of internal carotid artery, and under the diagnosis GDC embolization surgery was performed on six days after the onset. The patient has completely recovered and keeps his wellness untill now after six months from the surgery except for loss of left eye vision.

　We believe that the endovascular surgery using stents will become the prime modality for treatment of traumatic pseudoaneurysm in the future.
