頭頸部再建外科最近の進歩—myocutaneous flapの発展とその現況

Recent Advance in Head and Neck Reconst-ructive Surgery, Myocutaneous Flap: Its Developement and Clinical Application

　I．はじめに

　頭頸部再建外科（head and neck reconstructive surgcry）は，主として頭頸部腫瘍，特に悪性腫瘍切除に伴う大きな組織欠損を修復して，日常生活に最低限必要な機能を回復させるとともに，美容上の障害をも整えて社会復帰を可能にしようという立場から，従来，機能破壊的にならざるをえなかった頭頸部腫瘍手術の方向に一大変革をもたらし，過去20年間に著しい進歩をとげた。

　古くから用いられていたconventionalな局所皮弁から脱皮して，はっきりした栄養血管を軸に，タテヨコ比の大きな皮弁を安全に移動させることに成功したBakamjian^1）のDP皮弁やMcGregor^2）の前額皮弁は，再建困難な領域であった口腔咽頭の修復を可能にして，再建外科に大変革をもたらした。この頃から皮弁の血流動態に関する基礎的研究も盛んになり，Milton^3），McGregor^4），Smith^5）らによってrandom pattern flapとaxial pattern flapという2つの概念が生まれた。random pattern Hapでは，はっきりした栄養動脈がないから，subdermalおよびdermal plexusによって周辺からの血流に依存することとなり，短小な皮弁しかえられないのに対し，axial pattern flapでは，明らかな栄養動脈に沿ってデザインすることによって長大な皮弁がえられる，とする考え方である。DP皮弁や前額皮弁も血流動態の見地からあらためてその妥当性が確認され，groin flap^6）などの開発の基礎となったのである。

　Recent history of myocutaneous flaps is reviewed in the literature. Biological and clinical characteristics of myocutaneous flaps which are particularly concerned to head and neck surgery are delineated. They include pectoralis major, sternomastoid, trapezius and latissimus dorsi myocutaneous flaps.

　Fundamental technique of these flaps and some problems in their clinical application are summerized, confirming to many reports and the author's own experiences.
