Pectoralis major myocutaneous flapによる下咽頭再建の経験

THE RECONSTRUCTION OF HYPOPHARYNX BY USE OF PECTORALIS MAJOR MYOCUTANEOUS FLAP

　I．緒言

　頭頸部領域の再建には従来D-P flapなど有動脈性有茎皮弁が使用されてきたが，近年わが国でもmyocutaneous flapが注目され始めた。本法の最も有利な点は，一次的に再建できることであり，筆者らも各種の再建に本法を利用している。

　今回pectoralis major myocutaneous flapを用いた下咽頭の再建症例5例を経験したので報告する。

　Our method of reconstruction of the hypopharynx by use of a pectoralis major myocutaneous flap is described.

　Those cases include a patient with fistulae of the hypopharynx and necrosis in neck after total laryngectomy, and a patient who underwent total resection of the hypopharynx.

　It was found that myocutaneous flap has the great advantage for the immediate reconstruction of the hypopharyngeal defect, and of the prevention and closure of fistulae. There was no stenosis at the site of anastomosis in our series.
