 Radical removal of the maxilla leaves a defect of the hard plate that causes malfunction of articulation and mastication of foods. The authors employ a pedicled forehead-skin flap in order to reconstract the hard plate primarily after the radical removal. The technical details are discussed as follows:

 1) In cases of maxillary cancers where wide resection is required following irradiation and chemotherapy, simple suture of the palatine mucosa may fail to reconstruct the hard palate.

 2) This failure is dependent upon the disturbance of blood supply from the internal maxillary artery.

 3) The palatine mucosa can be successfully replaced by pedicled forehead skin flap.

 4) The total defect of the hard palate thus created by the operation is reconstructed by the lined split thickness skin graft.

 5) Forehead-skin flap without lining is also good enough to replace the total palatine defect when rotated 180 degrees and sutured to the residual oral mucosa.

 6) An excellent articulation is obtainable by reconstruction of the palate with a pedicled forehead-skin flap.
