上顎悪性腫瘍治療に伴う眼障害について—第1報臨床像

Ocular Lesions Caused by Treatment of Malignant Tumor in Maxilla:Part I : Clinical Pictures

I．はじめに

　上顎腫瘍，特に上顎癌に対して，通常手術とその前後の強力な放射線治療が行なわれるが，これに伴つて重篤な眼障害がほとんど必発する。この障害はしかもきわめて難治で，長期間患者を苦しめ，しばしば失明に至り，あるいは苦痛のために眼球摘出を余儀なくされる。

　われわれ^4）は先にこの障害について報告したが，その後の症例も加えて，臨床像，視力の予後などについてさらに検討した。

Eighty blepharoconjunctivitis, 68 diffuse su-perfical keratitis and various other lesions were observed within 6 months after the initiation of treatment in 80 patients of malignant tumor in maxilla. All had undergone irradiation of Co⁶⁰. Average dose was about 7, 500 r. Besides irradiation 75 had total maxillectomy and 3 had partial maxillectomy. Two had no operation.

Among the 23 patients survived long enough and could have full examination, 18 had corneal opacity, 16 had cataract and 7 had glaucoma. Various other lesions were also noticed in small number. Three needed enucleation.
