I．緒言
エスプンジア（アメリカ・リーシュマニア症）は，わが国では稀な疾患とされている，戦後の報告を見ても，河村1），酒井2），横川3）など数例が報告されているにすぎない。そしてほとんどが，臨床症状，既往歴などより診断しえたものであり，Leishmania原虫の証明はできていない。われわれは，10数年前より次第に進行してきた右鼻入口部潰瘍を主訴として来院した患者において，培養によりLeishmania原虫を証明することができ，Espundiaと確定診断しえたので若干の考察を加えて報告する。
A rare case of espundia (Lieshmaniasis americana) is reported. A man, aged 71, complained of lesion at the right anterior choana which gradually increased in size for the last 10 years. The patient underwent treatment for syphilis and tuberculosis previously with no improvement. With a history of having resided in Brazil previouslyand with other laboratory tests such as blood sedimentation and γ-globulin tests Leishmaniasis brasiliensis was suspected. The blood examination showed a positive Leishmania organisms.
The patient suffered symptoms of turbinal atrophy, septum perforation, soft palate atrophy and granulomatous epiglottis.
With intravenous administration of sodium antimonyl tartrate (stibnal) a marked improvement occurred with desication of the lesions.
