 A direction-changing apogeotropic positional nystagmus in a 29 year-old female with a spinal cord tumor is reported. This particular type of nystagmus is generally known to be seen in the supratentorial lesion or in bilateral labyrinthine lesions occurring at the same time in same degrees. However, our case suggests another possibility that this nystagmus could have been the result of simultaneous involvement of bilateral vestibular system at the brain stem where they are topographically closely located.

 The authors postulated that the foregoing situation took place at the vestibular nuclei in the present case as the result of compression by the cystic mass (neurinoma) in the dorsal part of the spinal cord at the level of the first cervical vertebra, which was reveald by later surgery.
