骨膜を使用する鼓室成形術とGelfilmによる橋部補修

TYMPANOPLASTY WITH THE USE OF PERIOSTEUM AND GELFILM

　I．はじめに

　筋膜を使用する外耳道保存型の鼓室成形術は，すでに現在，耳科手術として普遍的なものになつており，遊離皮弁使用の古典的鼓室成形術に対して，多くの点でその優位性を高く評価されているのは衆知のことである。

　しかし，その反面，病巣の徹底的清掃が困難であること，術後の外耳道や鼓膜の内陥，あるいはⅢ型において，筋膜の広範な癒着による聴力低下がおこりやすいなどで，遠隔成績からみると，意外に思わしくない結果を散見する。

　われわれは，過去3年間に経験した鼓室成形術について検討し，筋膜使用の外耳道保存型鼓室成形術で，術後穿孔率が20％前後を示したこと，Ⅲ型における聴力改善成績がきわめて悪かつたことについて反省し，その対策を考えた結果，昭和47年5月より，少数例ながら筋膜にかえて，骨膜を鼓膜形成材料として用い，また骨膜使用例中13例には，橋部を落としたものに，これを補修する目的でGelfilmを併用してみたので，その結果について報告する。

　Twenty cases of meatotympanoplasty by means of periosteal graft are reported. Gelfilm was used in 13 cases in order to reconstruct the missing portion of the removed posterior wall of the bony canal. Untoward reactions were seen in 2 cases with postoperative perforation and retracted pocket formation. Hearing improvements were established in 15 cases (75%).

　On the basis of preventing postoperative perforations and establishing hearing improvements, the authors regard the use of periosteum by far superior to that of fascial graft. The gelfilm was useful in supplying the defect of the bony canal wall and in avoiding adhesion of the graft to other structures such as the promontory and middle ear ossicles.
