癌による食道気管瘻について

BRONCHO-ESOPHAGEAL FISTULA DUE TO CANCER

　Ⅰ．緒言

　食道癌は嚥下障害，反回神経麻痺による膜声，嚥下痛，さらには癌が気管壁に浸潤し，呼吸困難などの症状を訴えて初めて受診する場合が多い。しかし通過障害や呼吸困難は食道癌の末期症状である。放射線治療や外科的治療の限界を越えた，いわゆる末期癌として姑息的療法に終わることが多い。特に食道気管気管支瘻を作ると，その管理・治療は非常に困難となる。喉頭癌，下咽頭癌の再発例にもこの問題が生じてくる。

　呼吸困難および飲食時の咳嗽を主訴とした胸部食道癌2例，下咽頭頸部食道癌再発例2例，ならびに喉頭癌全摘後の頸部食道部再発例1例を含む計5例を中心に，癌による食道気管気管支瘻の管理・治療の困難性，特に気道の確保，栄養，二次感染すなわち嚥下性肺炎の予防，疼痛に対する処置，さらに姑息的手術などの問題を検討した。

　The authors report 5 cases of esophageal cancer in which 2 cases complained of difficulty of breathing and swallowing accompanied by persistent coughs the effect of cancerous involvement of the thoracic esophagus; 2 cases of recurrent cancer of the cervical esophagus; and, 1 case of recurrent cancer of esophagus following earlier total laryngectomy due to cancer of the latter organ. Varius aspects of the management of bronchoesophageal fistulae, the difficulty of their treatment, particularly, for proper maintainance of an adequate air-way, the upkeep of the nutrition, prevention of secondary invasion of deglutition pneumonia, the need of pain relief and other symptomatic treatments are discussed.

　For proper maintainance of airway laryngeal intubation was employed; and, for nutritional care the need of intravenous hyperalimentation was emphasized.
