糖尿病と喉頭全摘出術

DIABETES MELLITUS AND TOTAL LARYNGECTOMY

　I．はじめに

　近年，老齢入口の急増，医学の進歩による糖尿病患者の寿命の延長，糖尿病の遺伝因子を持つた子孫の増加，食生活の豊かさと肥満傾向などにより，糖尿病は次第に増加している。栗原^7）によると，わが国の死亡統計でも，男子では昭和29年以降，女子では昭和22年以降糖尿病による死亡が増加している。また，村地^11）によると，糖尿病研究班による集団検診では40歳以上の陽性率が，昭和32，33年には4.6％であつたのが，昭和39年には7.2％に増加している。耳鼻咽喉科領域においても，糖尿病を合併した患者は増加するであろうし，それらの患者に対して手術を行なう機会も増えてくるものと考えられる。

　われわれは，最近，糖尿病を合併する喉頭癌患者11例に対して喉頭全摘出術を行なつたので，術前・術後の管理の重要性について報告する。

　During the period of 5 years, from 1969 to 1973, it was necessary to perform total laryngectomies under local anaesthesia on 11 patients, who were suffers of diabets mellitus.

　Postoperative complications were ineveitable in 7 cases, with hyperosmotic non-ketotic coma in 1 case, fistula formation in 3 cases, urogenital infection in 2 cases and hypoglycemia in 1 case.

　In order to undertake operative procedures in diabetic patients a full cooperation of the Internist should be sought with. And a close postoperative follow-up should be adamant.
