術後性上顎嚢腫の知見補遺

THE POSTOPERATIVE CYSTS OF MAXILLA

　I．はじめに

　いわゆる手術後性頬部嚢腫は久保猪之吉教授^1）により初めて昭和2年に提唱された。その後，現在にいたる45年間に多数の報告と研究が発表され，本症の病態は次第に明らかとなつてきた。またこの間に慢性副鼻腔炎の保存的療法は抗生物質，消炎酵素剤，抗ヒスタミン剤，副腎皮質ホルモンなどの利用により大きな変化をとげ，その手術的療法も現今実施されている種々の術式へと発展した。典型的な手術後性頬部嚢腫は上顎洞根本手術をうけた10ないし20年後に頬部腫脹，疼痛，歯痛などの症状で発病するとされ，これが手術後の上顎洞内に発生した嚢腫の圧迫性拡大によるものであることは衆知である。著者はのちにのべるごとく，本症は術後の治癒機転が障害された一現象であるとの見解に達した。またすでに笹木^2），藤田^3），岩本^4）などの見解にみるごとく，本症の名称は術後性上顎嚢腫であるとするのが妥当であると思うため，その名称を用いた。この報告の目的は過去45年間の本疾患治療面での進歩や発展が本症の臨床に対していかなる影響を与えたかの点につき検討し，同時に現在まで発表された病因論につき若干の考按を加えることである。

　Two hundred twenty six cases of postoperative maxillary cysts that were met with during the period of 5 years from 1966 to 1970 are studied. The cysts occurred twice as often in the male as that of female, the right side slightly more affected than the left and more than two thirds of the cases were seen between the ages from 30 to 40. The cysts were found to be essentially either mucoor pyoceles and the vulnerable period was 10 to 20 years after the initial sinus surgery.

　The etiology of the cysts appeared to be initiated by closed cavity in which the infected sinus mucosa was left behind in a small piece at the time of the initial surgery or those that grew post-operatively to cause the closure of the sinus cavity by formations of scar tissues. The possibility of the cyst formation may be avoided or reduced by preventing postoperative infection and by controlled postoperative care following the initial sinus surgery.
