巨大な歯根嚢腫の一症例

A CASE OF AN EXTENSIVE RADICULAR CYST

　顎部嚢腫は比較的よくみられるもので，その歯牙への関係から歯性と非歯性とに分けられる。非歯性のものには外傷性血管腫のごときものと，胎生期の上皮が縫合線内に迷入して生ずるものとがある。歯性のものには，衆知のごとく，濾胞性嚢腫と歯根嚢腫とがある。耳鼻咽喉科領域の大部分の教科書および文献においては嚢腫として表現されているが，今川^1）のごとく嚢胞とする用語もある。歯根嚢腫と歯根嚢胞とは同一のものであるから，以降，大勢に従つて嚢腫を用いる。歯根嚢腫は齲歯の慢性歯髄炎に歯根部肉芽腫が発生し，これにある種の上皮が埋入して上皮性肉芽腫を形成し嚢腫状に至つたものをいう。この症例は左上顎第2小臼歯より発生したと思われる歯根嚢腫に関するものであり，この歯根嚢腫は後述のごとく，左上顎洞全体に拡大し，犬歯窩にて上顎洞前壁を破り左頬部腫脹を来たしたものである。

　A 49-year-o1d male with the swelling of the left cheek of about a month's duration was seen. The cyst arose from the root of the second premolar tooth in the left maxilla and it occupied the entire left maxillary sinus. Microscopic study of the cyst wall and chemical analysis of the fluid content were carried out The origin of the fluid seemed largely inflammatory and the accumulation of the content enlarged the cyst.
