滲出性中耳炎成立機転に関する一考察

PATHOGENESIS OF SEROUS OTITIS MEDIA

　I．はじめに

　最近，滲出性中耳炎が急激な増加を示しており，特に小児の場合その発生頻度が大で学校検診においても伝音性難聴の大部分を占める傾向にある。

　しかも，その貯留液は成人のに比して粘稠度が高く治療に困難を伴い，一部ではこれを "gallert-erguss" を伴つた中耳カタル，あるいは "glueear" と称して特別な関心をはらつている。

　また，本疾患の小児における急増の理由として多くの報告者が，1）不用意な抗生物質の乱用，2）口蓋扁桃，アデノイドの積極的除去を行なうことが少なくなつたことをあげている。

　疾患が遷延すれば当然に聴力損失を残し，あるいは癒着性中耳炎，青色鼓膜などにも進展して手術の対象とせざるを得ぬと強調している。

　本疾患にみられる貯留液には漿液性の場合と，粘稠なにかわ様の場合とがあり，さきに湯浅^45）はこれら疾患の耳管通過性を測定して発表したが，中耳腔内に貯留液を認めながら耳管通過性は軽度に障害されるか，あるいは正常値に近い症例もあり，特にそれが粘稠な貯留液を示す場合に多く認められた。

　一方，Zöllner（1936）^46）も本疾患の大部分において耳管の閉塞例を認めず，炎症の関与を強調したのを初めとして，Suehs（1952）^38），Senturia（1962）^34），Schuknecht（1967）^33），Rankin（1970）^27），Lim（1970）^19）らも耳管の閉塞を認めない症例の存在することを記載している。

　かかる傾向から本疾患の病像を再解析し，またその成立機転についても実験的に検討することは意義あるものと考え，われわれは数年来本疾患の耳管機能，チューブ経鼓膜的挿入の効果などを観察するとともに，罹患時，および動物実験において作られた中耳腔粘膜につき病理組織学的追求を行なつたので，ここにその大略を報告する。

　The authors made a clinical and histopathological investigations of serous otitis media which is rapidly increasing in number in recent years, with discussions of its pathogenesis. In cases of serous retention, increased permeability of the serum at the capilary wall in the mucous membrane of middle ear cavity is suggested and edema in the epithelial layer is found in the experiments.

　In the cases of mucous retention the presence of increased number of mucous secretory cells are suspected in the biopsies.

　For the treatment of this disease the intubation through the tympanic membrane combined with adenoidectomy appears the most favorable method; 89.8% of cases are completely cured.
