 In order to clarify the occurrence of secretory otitis media (SOM) in school children, 347 children who entered the elementary school in 1976, 1978 and 1979 were longitudinally investigated every spring and fall for six to three years.

 1) The primary SOM was found in pupils of the first, second and third grade and their incidences were 0.5-4.0% in each examination. No case, however, was noted in the upper grade pupils.

 2) The incidence of the primary SOM for 3 years was 9.9%.

 3) The incidence of the primary SOM was higher in children with a past history of acute atitis media (AOM) (16.9%,) than in those without (6.1℃). This result showed that the history of AOM seems to have a close relationship with SOM.

 4) The important factors for the occurrence of primary SOM seem to be included a history of AOM and immunological immature condition in the middle ear.
