I．はじめに
1922年，Werner, Schultzが急激な高熱をもつて始まり，粘膜の壊死，時に重篤な扁桃炎をきたし，末梢血中から顆粒球が消失し予後のきわめて不良な原因不明の敗血症様疾患を報告し，これに本病名を付して以来，種々の原因による症例が発表されてきた。本症の原因として，種々の薬剤が関与している場合が知られている。1954年，Moeschlin以来，多くの研究者によつて，白血球減少症の発生メカニズムに対して薬物アレルギーないしアナフィラキシーが発生の根底にあると考えられ，いわゆる免疫学的反応の関与が論ぜられている。われわれは抗生剤によると思われる本疾患を経験したのでその経過を報告し，合せて本邦文献における報告例を比較検討してみた。
The clinical progress of agranulocytosis due to administration of antibiotics is reported; 49 similar cases are collected from the literature of this country. The diagnosis of this condition is based on clinical symptoms such as high fever and sore throat or findings of specific ulceration in the oral cavity, pharynx or tonsils. Such lesions are generally covered withdirty yellowish-gray or dark-brownish furs. The blood picture is found with a decreased or loss of granulocytes.
The treatments consist of (1) removal of the cause, (2) restitution of the blood count, (3) stimulation of the bone marrow activity and (4) prevention of reinfection. Other symptoms are treated symptomatically.
The prognosis is dependant upon the presence or absence of complications regardless of the white blood cell count.
