　Ⅰ．緒言

　著者らは舌咽神経痛の手術療法による一治験例を報告したが^1），その症例は2年2カ月後疼痛の再発を生じた。初回の頸部手術による舌咽神経切断より3年11カ月後に口腔咽頭内からのapproachにより同側の舌咽神経を切除し，再び疼痛の消失をみたのであらためて記載したい。

　特発性舌咽神経痛とは，とくに原因とみられる器質的病変がなく神経痛様激痛が患側の舌根部・口蓋扁桃・中咽頭側壁・上咽頭の一部から耳にかけて生じる。痛みの中心は個体差があるが，舌根・扁桃部に誘発点があり，綿棒などで刺激すると痛みを起こすことができる（trigger zone）。また嚥下・会話・せきによつても痛みが誘発されるきわめて苦痛の多い疾患である。

　A 35-year-old house-wife developed glossopharyngeal neuralgia again 26 months after the ninth nerve sectioning with the cervical approach. This time, the nerve was sectioned oropharyngeally (transtonsillarly) and the patient had a complete relief of neuralgia attack. The author described the surgical technique of this operation and discussed the surgical treatment of idiopathic glossopharyngeal neuralgia.
