同種筋膜移植実験について—中耳における移植免疫学的特殊性に関する考察

EMPLOYMENT OF HOMOFASCIA IN TYMPANOPLASTY

　I．はじめに

　Otosclerosis，Otitis media chronica，Perforation of the tympanic membraneなどの手術に人工材料，異種・同種組織^{1）2）3）4）}を補修材料として使用してもなんらのtroubleもなく良好な成績を得ている。しかしながらかかる物質，組織を用いて，なぜ拒否，異物反応が起きないのか，その理由については研究されていない。

　私どもは優れたautograftを選ぶには，同名のhomograftを移植して拒否されない組織が最良であろうという逆説的な考えにたつて，現在tympanoPlastyで用いているfascia^5）について検討した。そこで臨床的にhomofasciaを鼓膜の再建材料として使用^6）してみた。さいわいhomofasciaは拒否されることなく1年以上生着している。この結果fascia自体の性状に関係があるのではないかと考え，まずgraft-sideの条件について調べた。fasciaの性状はdiffusion栄養下で同種移植のできる角膜組織と非常に類似している。つぎに抗原性の検査として，homofasciaを中耳以外の部位に移植してみると，臨床的および病理組織学的に明らかな拒否現象が証明された。かかるgraft-sideの観察結果から，当然recipient-sideの条件について検討する必要が生じた。今回の臨床的移植実験の対象になつた5例はいずれも分泌物がなく，ほぼ正常に近い中耳粘膜であつた。つぎに移植免疫で抗体産生の反応中枢である所属リンパ節に対する解剖学的関係が，同種組織の移植可能な前眼房^7），睾丸^8），hamsterの頬嚢^9）などと類似性を示すことが文献的に考察された。以上のgraftおよびrecipientとの相互関係から「正常の中耳は移植免疫学的に特殊な部位である」と推論するに至つた。

　For covering the gap made in the operation of otosclerosis and of perforation of the tympanic membrane due to chronic otitis media, artificial substances or homologous tissues obtained from the same or different specie may be employed without the usual signs of rejection. However, the underlying factor supporting this reality has not been investigated.

　The authors performed a tympanoplasty employing homofascia with satisfactory results even at the present date, one year after the operation. A study on this subject showed that when the nutrition of the grated-tissue is dependent on diffusive means, such as the case of corneal graft, the procedure appear to be successful. But, when homofascia is grated to other parts of the body, it is eventually rejected.
