S状洞の走行異常に關する臨床的並に解剖学的研究

緒言

　乳樣突起削開に際して注意せられているものの一つにS状洞の走行異常特に前進と浅在がある。従来これは中頭蓋窠底の下降とともに所謂危険性側頭骨として特別な考慮が払われ，臨床的並に解剖学的に調査研究が行われている。その主な研究対象は，乳樣蜂窠を削開する前に危険性側頭骨を予知する種々の目標の信頼度を追求確立することであり，更に根本的な成因について誼索する研究である。殊に成因については先天説と後天説が対立していて，これが解決には更に多角的観察の必要性が痛感せられる。

　余は最近施行した乳樣突起削開260例中に遭遇したS状洞の走行異常例について，種々の方面から成因の考察を行い興味ある成果を收めたので茲にその大要を報告し諸家の御批判を仰がんとするものであるが，更にS状洞の前進と関係ありとせられている数種の予知目標を計測的数値によつて明らかにするため，解剖学的観察を行つたので，それ等の成績を報告し参考の資料に提供したい。

　SAKAGUCHI states that evidences that point towards favoring influences that are external as the cause of abnormality with which sigmoid sinuses are found relative to the mastoid cells are strong. Out of 260 mastoidectomies performed 57 cases of positional abnormality were encountered. As to thedegree in the angle, which is formed by the posterior wall of osseous canal and the surface of mastoid bone, for possible criteria in prognosticating the abnormality, there appeared to be no ground of proof.
