　緒言

　頸靜脈球部附近の血栓症に對しては，從來Grunertの手術が行われているが，此の手術が患者に與える身體的負擔は甚大である．これがもつと簡單な方法で行えるならば非常に益する所が多い．此點について鳥居教授は昭和24年10月24日の北陸地方會に於て1案を提案しておられる（目耳鼻，52巻，8號，296頁，昭25）．

　餘は最近，廣範なる横竇及びS字状竇血栓が頸靜脈球部及び内頸靜脈上部に及んでいた重篤なる症例に遭遇したが，頸靜脈球部及び内頸靜脈上部に對しては，同教授の提案された方法に準じた方法を實行することにより治療せしめ得たので聊か所見を述べたいと思う．

　In an extensive thrombosis of superior portion of the jugular vein involving the transverse and sigmiod sinuses as well as the bulb, in a patient, a man aged 18, Kojima treats successfully, without resorting to Grunert s operation, by a method advanced by Professor Torii which employs jugular i rrigation and parenteral use of penicilin. A Nelaton' s catheter is inserted in the jugular through the neck and the the thrombosed vein is irrigated out up to the region of the bulb. The author stresses that this method should be given a fair trial before relying upon the usual Grunert's operation.
