一粘液浮腫患者の異常頭位眼球震盪—異常頭位眼球震盪の発現機轉

　緒言

　異常頭位眼球震盪は第一次欧洲大戰後独乙その他北欧諸國の内耳機能研究者により注目せられた一種の眼球震盪であるが，今日尚其発生原因も臨床上の價値も明かでなく從つて一般に知られるところも比較的少い．唯かゝる，正常頭位では眼球震盪を起さず，頭部の側位又は仰位で初めて眼球震盪の発現する如き疾患として，多発性硬化症，脳腫瘍，頭蓋底骨折，震盪症，進行性麻痺症，動脈硬化症，中毒，仮性脳膜炎，原発性交感性眼炎，限局性内耳炎，及びメニエル氏病等が知られてゐる．殊に後頭蓋窩の腫瘍では頻発し，Nylénによると90％に見られると言う．又Nylenはその原因を腫瘍により惹起された後頭蓋窩の脳圧上昇と血液及び淋巴液の循環障碍が，更に頭位を一定度変化する事により，前庭神経核に強力に影響を及ぼした事に帰してゐる．即ちこの見地からするならば，脳腫瘍以外の他の疾患でも，頭蓋内圧の変化及び頭蓋内循環障碍を起し得る場合には，そこに異常頭位眼球震盪の発現が考へられてよいのである．私は最近粘液浮腫の一患者に於て異常頭位眼球震盪を認め得たので，その所見を併せて考察の一端を述べて一般の御批判を仰ぎ度い．

Matsumoto reports a case of nystagmus which occurred in a myxedematous patient on postural changes of head positions. The chief complaints of the patient, a woman 59 years of age, were impaired hearing, tinnitus and vertigo. Physical examination revealed a state of myxedema and generalized edema of which the patient related the history of 8 years standing. Thyroid medica-tion was tried, after which the state of genera-lized edema improved for a time but, only to relapse again. Since then the patient's symptoms has been taking aIternate courses of remissions and relapses. On manifestation of bilateral nys-tagmus the author considers the uuderlying cause the result of edematous changes in the cellular structure of the inner ear whereby, disturbance of balance on osmotic pressure is created in the endolymph.
