Primitive Trigeminal Arteryが関与していた三叉神経痛の2例

Trigeminal Neuralgia associated with Primitive Trigeminal Artery: Report of two cases

I．はじめに

　特発性三叉神経痛は，触覚や痛覚の低下または脱失が認められない三叉神経知覚枝領域にわたる発作性激痛である．本症は，三叉神経が脳幹にはいる部位＝root entry zoneにおいて，屈曲した動脈あるいは静脈により圧迫されて起こる場合が多いと言われている．1976年Janetta^5）により，顕微鏡下にこれらの圧迫血管を剥離し，筋肉片あるいはスポンジを挿人する方法，神経血管減圧術が確立されてから，本症の治療成績は革命的に進歩した．当院においても，昭和56年より本疾患に対する治療を手がけ，現在までに131例の手術を行い，良好な結果が得られている．その中で，胎生期遺残動脈が関与していた稀な2例を経験したので，症例を呈示し，若干の文献的考察を加える．

Abstract

　Two cases of trigeminal neuralgia associated with the primitive trigeminal artery are reported. From 1981, the authors have treated 131 trigeminal neuralgia patients with microvascular decompression. Among them, we encountered two rare cases of trigeminal neuralgia associated with the primitive trigeminal artery (PTA) and its variant (PTAV).

　Case 1 is a 79-year-old woman who was admitted to our hospital due to pain of maxilla and mandibula.
