I.はじめに
2.5cm以上の巨大脳動脈瘤は全脳動脈瘤中の7.4—13%6,11-13)の発生率が報告されているが,近年micro—surgeryの導入および巨大脳動脈瘤用の特殊クリップの開発にもかかわらず,なお手術死亡率は15-21%8,11-13)の報告があり,また手術による合併率も高く,その治療成績は満足すべきものではない.特に65歳以上の高齢者の巨大動脈瘤症例での外科的治療の決定にあたっては,手術のriskが大きいだけに,非手術例の巨大動脈瘤患者のnatural historyを正確に把握することが重要であると考えられる.今回,非手術例の巨大脳動脈瘤症例28例のnatural historyにつき,脳血管写,CT所見および臨床症状を参考にして検討した結果,興味ある知見を得たので,その結果につき報告する.
The outcome of a consecutive series of 28 patients with giant aneurysm who had been followed without surgery from one month to 12 years after the diagnosis was made, are presented with reviewing their radiolo-gical and clinical features.
Symptoms and signs were directly or indirectly attri-butable to the intracranial mass effect and nine patients (32%) presented subarachnoid hemorrhage. Subarach-noid hemorrhage was frequently associated with in-traventricular or intracerebral hemorrhage, a poor clin-ical grading at admission and a high mortality.
