Lipo Prostaglandin E1 Administration in the Acute Stage Following Subarachnoid Hemorrhage

I．はじめに

　脳動脈瘤破裂によるくも膜下出血後の遅発性脳血管攣縮は，破裂脳動脈瘤患者の臨床像および予後に多大な影響を及ぼす．脳血管攣縮に起因する脳虚血症状の発現に関しては，血管狭窄による血流減少とともに血小板凝集による血栓形成が大きく関与すると考えられている^15,28,33）．

　prostaglandin（PG）E₁は血管拡張作用および血小板凝集抑制作用を主たる薬理作用とするprostaglandinである^35,37）．われわれは，従来のPGE₁製剤に比べ血中半減期の長いPGE₁含有脂肪乳剤Lipo-PGE1を脳血管攣縮由来の脳虚血症状を発症した患者に投与し，脳血流の増大と臨床症状の改善が得られたことを報告した^21）．

　Prostaglandin (PG) E₁ is a potent vasodilator on the peripheral vessels and also has an inhibitory action of platelet aggregation. Thus it is expected that PGE, may be used for the treatment of cerebral vasospasm due to aneurysmal subarachnoid hemorrhage (SAH).

　Lipo-PGE₁ lipid emulsified PGE₁ less destroyed in the lung, has much longer half life time in the circulation than PGE₁ which is rapidly inactivated in the lung.

　A pilot study, examining the clinical effect of Lipo-PGE₁ on cerebral vasospasm, was performed.
