急性期脳虚血性浮腫に対する外科的治療—特に海馬回切除術の有効性についての検討

Unilateral Inferior Temporal Lobectomy with Hippocampectomy for Acute Ischemic Brain Edema

I．緒言

　内頸動脈や中大脳動脈などの脳主幹動脈の閉塞，くも膜下出血後の高度なvasospasmの症例では，虚血性脳浮腫のため症状が進行性に増悪し，内科的な保存的治療では救命できない症例に遭遇する．われわれはこのような急性虚血性脳浮腫症例に対して内および外減圧術を施行した10症例の手術方法および転帰などを検討し，若干の知見を得たので，その結果について報告する．

Intractable brain edema with hippocampal gyrus her-niation through the incisura of the tentorium is a fatalcomplication of the main cerebral artery occlusion andsevere vasospasm following subarachnoid hemorrhage.

We presented 10 operative cases of ischemic brain edema,3 due to middle cerebral artery occlusion, 2 due to internalcarotid artery occlusion and 5 due to severe vasospasm,discussing the surgical indication and prognosis in severeischemic brain edema cases.
