I．はじめに

　神経鞘腫は成人において最も頻度の高い脊髄腫瘍であり，欧米では全脊髄腫瘍の20-30％^7,8），本邦ではさらに高く40％程度^3,4）を占めている．しかし，脊髄神経鞘腫が髄内に存在することは極めて稀であり，われわれの検索し得た限りでは31例を数えるのみである^4,10）．また本腫瘍が大嚢胞（macrocyst）を形成することも脊髄においては稀である^11）．

　今回われわれは，四肢麻痺で発症した頸髄髄内外多？胞性神経鞘腫を経験した．この症例を呈示するとともに近年脊髄疾患診断に重要な位置を占めてきた磁気共鳴画像法（以後MRIと略す）^6）の本症例診断に果たした役割などについても考察を加え報告する．

This report examines a case involving a huge cystic cervical neurinoma existing extra- and intramedullary. The histogenesis of intramedullary neurinoma, patho-histogenesis of large intratumoral cyst are discussed. The role of Magnetic Resonance Imaging (MRI) in the diagnosis of spinal tumors is also discussed.

A 23-year-old male was admitted to our ward or hav-ing tetraparesis. On admission, spastic tetraparesis and sensory disturbance below level C 2 were noticed.
