急性水頭症で発症した脳内原発性悪性リンパ腫の1例

A Case of Primary Malignant Lymphoma of the Brain Associated with Acute Hydrocephalus

I．はじめに

　原発性頭蓋内悪性リンパ腫は，全脳腫瘍中，O．5％—1.5％前後^6-8），本邦の脳腫瘍全国統計でも0.9％^14）と，比較的稀とされてきた．しかし，近年の画像診断の進歩，および種々の免疫能低下状態におかれる患者が増加するに従い，その頻度は急増する傾向にある．その診断において，近年の免疫組織学の進歩にともない，単にT細胞系，B細胞系の同定のみならず，それぞれの系の中の，どの分化段階での腫瘍化かを推定できるようになりつつある．今回われわれは，急性水頭症で発症し，髄液細胞診で悪性リンパ腫と診断された1例を経験した．頭蓋内には腫瘤陰影を認めず，臨床的には脳原発性髄膜播種性悪性リンパ腫と考えられ，その免疫組織学的検査結果を含めて，若干の文献的考察と共に報告する．

A case of primary cerebral malignant lymphoma associated with hydrocephalus is reported. The patient was a 54 year-old male who enjoyed good health until the onset of headache and vomiting 4 weeks before admission.

His consciousness was alert and neurological ex-amination revealed severe papilloedema with retinal hemorrhage. No lymph node or abdominal tumor en-largement were noted. CT scan and MR images re-vealed no abnormal lesion except mild ventriculomega-ly. CSF study revealed mild elevation of protein and sugar and cell count was 66/3.
