I.はじめに
persistent primitive trigeminal artery(PPTA)は胎生期の内頸・脳底動脈吻合血管(carotid—basilar anastomosis)の遺残動脈の1つであるが,脳動脈瘤などの血管異常を高率に合併することで注目されている.しかしPPTA本幹部に発生した動脈瘤の報告例は極めて少なく,しかもその剖検例による検討は認められない.われわれはPPTA本幹部に発生した動脈瘤の1例について,臨床的および病理組織学的検索をし得たので文献的考察を加えて報告する.
A case of an aneurysm on the persistent primitive trigeminal artery (PPTA ) is reported.
A seventy-five-year-old woman with subarachnoid hemorrhage was admitted to the Hospital six hours af-ter the onset. She was lethargic and had stiffness of her neck with severe headache and vomiting. Computed tomographic examination showed marked subarachnoid hemorrhage in the basal cisterns, especially in the ambient cistern on the right side. Cerebral angiogramrevealed the right PPTA having a saccular aneurysm on the trunk. Her advanced age and the special location of the aneurysm did not allow direct operation in the acute stage.
