I．はじめに

　脳腫瘍，特に悪性脳腫瘍に対する治療としての放射線治療の占める役割は大きい．現在，照射法としては，Telecobalt,Linac,Betatronによる遠隔照射が一般的である．一方，密封小線源を定位脳手術により腫瘍内に植え込む組織内照射や，手術腔内に線源を挿入する腔内照射は，一部の施設においてのみ行われているにすぎない．その理由として，手技の繁雑さ，術者の被曝，患者管理の問題とともに，線源の確保や特殊な設備を必要とすることなどが原因として考えられる．

　ところで密封小線源治療では線源が腫瘍中心部に位置するために，遠隔照射とは比べものにならない大線量を選択的に腫瘍組織に投与することが可能である．そのため正常組織の損傷を少なくし，治療可能比をあげるという利点がある．したがって，悪性脳腫瘍に対する組織内あるいは腔内照射は有力な放射線療法の1つと考えられる．

Three patients harboring glioblastoma were treatedby fractionated intracavitary radiation using remoteafterloading system. For this therapy, a specialintracranial applicator was designed and used.Radiation schedule was planned to deliver 3000 rads tothemargin of the tumor in 3 fractions over 10 days. Theadvantages of this treatment are that completeelimina-tion of all unnecessary radiation exposure ispossible,and that placing a radioactive source in the tumorprovides maximal delivery of radiation to the tumorwith relative sparing of surrounding normal brain.
